Camper Forms:  All forms are due by May 1, 2013.
 
Health History – basic health information, including immunizations. If you were enrolled in Summer 2012, most  information will pre-populate.   
 
Physician Examination (PDF) – print, give to your child's physician to complete and sign, then fax back to 301.686.8611.
 
Camper Sunblock – complete and submit online.
 
Authorization to Medicate – only required for campers taking medication at camp. Print form and bring to Health Room with medication 
 
Statement of Understanding – complete (you and camper) and submit online.
 
Camper Arrival/Dismissal – complete and submit online.
